Regarding name change in Registration form of my ward
Due to my marriage i changed my name from Reeta Minz to Reeta kujur but after few years as my all documents is with name Reeta Minz I changed my name from Reeta kujur to Reeta Minz in all documents like pan and aadhar card.Now my ward is studing in DAV Nandraj public school bariyatu road ranchi in class IX and now in his Registration form school write as Reeta kujur in refrence to his Birth certificate form but I already change my name by affidavit as Reeta Minz. Hence what can I do to change my right name in my ward registration form . Kindly help
